This form must be notarized before any piercing procedure can be performed on a minor, no matter where you go in the state of Flordia. 📍 Need a Notary? Notaries are readily available at most local UPS Stores, post offices, and insurance agencies throughout Lakewood Ranch, Sarasota and Bradenton.
